Chronic Care Management Program:

Ritchie Regional Health Center is hard at work developing a Chronic Care Management Program for Medicare recipients. In collaboration with DSHI systems, this telehealth program connects you with a case manager, who will work with you and your care team to provide monthly phone calls to answer your questions, discuss your on-going issues, provide educational materials, and to work towards your overall wellness. Additionally, we can help coordinate your visits with other doctors, facilities, and testing organizations, work with you on your medications, all provided to you in a comprehensive care plan.

This program requires an initial in-depth physical exam and the diagnosis of at least two chronic conditions. Chronic conditions include Alzeimer’s disease & Dimentia, Asthma, Cancer, CAD, COPD, Mental/Behavior Health, GERD, Diabetes, PTSD, Stroke, Arthritis, PVD, and others.  If you are interested in finding out more about this program, speak to your RRHC care team, or call (304) 643-4005.

*Medicare covers most costs of CCM. Talk with your insurance company directly to find out what your co-payment will be.
